Patient Safety

Carleton Place District and Memorial Hospital is committed to your safety. By working together with physicians, nurses and other hospital staff, you can lower your risk of injury and make your hospital stay as safe as possible. Here is how you can help us make your stay a safe and positive experience.

Ask questions

Feel free to talk to your doctor and nurse about any concerns. We welcome your questions. Ask questions if you do not understand. It is your body and right to know. If you think of questions when your doctor or nurse is not present, write down your questions so that you can ask at a later time. Ask for an interpreter if you are hearing impaired or if English is not your primary language.

Get the most from your treatment

Ask your doctor and nurse about your treatment plan. Make sure that your understand and agree with that plan. Ask a family member or friend to listen with you when diagnosis, treatment plan, test results or discharge plans are explained. If equipment or tests are ordered for your care, know why they are ordered. Question anything that seems unusual or different from what you were told.

Identify yourself

Wear your hospital ID bracelet at all times. If your bracelet comes off, ask someone for a new one. Check the information on your hospital ID to make sure that your information is correct.

Help prevent the spread of germs

Be aware that hand washing is the best way to prevent the spread of germs. Practice good personal hygiene. Ask friends and relatives who have colds and respiratory symptoms not to visit you in hospital. Get vaccinated. Flu and pneumonia vaccines can help prevent illnesses.

Get the most from your medicines

Tell your doctor and nurse about all medicines you are taking including vitamins, herbal remedies and over the counter drugs Bring your medications to the hospital on admission. Once the doctor or nurse has recorded your medications, send them home for safe-keeping Let your doctor or nurse know if you have any allergies or have had any reactions to drugs, food or latex

Protect yourself from a fall

Ask for help when getting out of the bed, especially at night. The hospital is an unfamiliar place and most falls occur when patients try to get out of bed on their own to go to the bathroom. Make sure you know how to use the nurse-call button. Make sure there is adequate lighting and keep your glasses within reach. Wear shoes/slippers with good support and rubber soles to prevent slipping. Ask the physiotherapist to show you how to use your walker or any assisting device safely. Alert staff to any spills immediately.

Personal Items

Pay careful attention to where you place your dentures, hearing aids and eyeglasses, as they are all important to your comfort and well-being. Dentures are best stored in a cup. Glasses and hearing aids are best kept in a case with your name on it.

Prepare yourself for discharge

Make sure you are clear about discharge instructions including medicines you need and information about a follow up visit. Keep a journal of the physicians, tests and hospitals you visit so you can update us at your next visit. Continue your safe practices at home, talk to your doctor and pharmacist if you have questions. Ask for a home assessment by an occupational therapist if you need grab bars, a hospital bed or ramps installed at home. Keep a phone near your bed if you need help. Quit smoking, we can help you with that. Be cautious if you have oxygen equipment at home. It is highly flammable.
