CPDMH’s Patient Relations service is in place to support you, our patients and families within Carleton Place Hospital. Our goal is to provide you with care and caring than enhances the patient’s overall experience.

We are available to assist you with any questions, concerns or compliments you wish to express.

If you are a patient at the hospital, you should contact the Patient Care Manager on the unit you are located to discuss the care you are receiving.

If you have been discharged from the hospital, please provide us with your feedback regarding your stay. There are several options available to contact us.

Patient Surveys:

  • “We Care What You Think” surveys are available in every unit in the hospital
  • NRC Picker Survey - After being released from the hospital, you may receive a survey in the mail. Carleton Place Hospital participates in continuous patient satisfaction surveys and the Board of Trustees reviews the results of these surveys every three months. Your responses help us improve our quality of care.OR

Email - Please send an email toinfo@carletonplacehosp.com. This will allow you to send any questions, comments or concerns to the hospital. If you would like to be identified, please include your name and contact information.

Letter - Send to Patient Relations, 211 Lake Avenue East, Carleton Place, Ontario K7C 1J4. Upon receipt of your compliment or concern, all verbal and written concerns will be acknowledged within 72 business hours of receipt, indicating the initial course of action and an expected period of investigation and response. Patient feedback is key to evaluation of services and ensuring change that enhances quality care.
