We are conveniently located on the main floor. Our department phone number is 613-253-3804.
- General X-ray including IVPs.
- Ultrasound – Abdomen, Pelvic, Obstetrical, Shoulders, Scrotal, Carotid Dopplers, Leg veins, and Thyroid examinations

Please call 613-253-3803 to book an appointment for your Diagnostic Imaging procedure.
Please notify our staff if you are diabetic and require an early morning appointment.
If you require an examination not performed at Carleton Place and District Memorial Hospital, our friendly staff will be able to assist you.
You will need to bring the following with you to your appointment:
- A completed requisition. This requisition is your prescription for your exam. It must include the date, your name, your date of birth (DOB), the examination requested, clinical information, physicians name and address and a physicians signature.
- Any relevant previous reports or images taken outside of the Carleton Place and District Memorial Hospital.
- Your OHIP and/or Carleton Place and District Memorial Hospital card.
Our clerical staff will call you prior to your ultrasound appointment to confirm the date and time of your appointment. Our friendly staff will also remind you of any preparations required.
Arrive 20 minutes prior to your appointment time to our Registration Department located on the main floor. Please wash your hands upon entering the hospital. You will be asked to take a number and have a seat. Please notify the clerical staff if you are attending a pelvic or obstetrical appointment and have a full bladder.
Once registered, you will be directed to the Diagnostic Imaging department. Our courteous staff will ask for your hospital card and requisition. Our department uses a double verification system. You may be asked to state your name, DOB and/or address. This information will be verified with your hospital card and requisition.
Before the examination you must notify staff if:
- You suspect or know you are pregnant
- You have ever experience a reaction to a previous injection of contrast media
- You are diabetic
- You have kidney disease
You may be asked to change into a gown and to remove jewelry for your examination. You may prefer to leave jewelry at home and attend your x-ray appointment dressed in clothes that do not contain zippers or buttons.

YOUR RESULTS
Our Medical Radiation Technologists, Medical Sonographers and Clerical Staff are pleased to assist you and answer any questions you may have about your medical imaging procedure but you will not receive results from the performing technologists.
Examinations are reported by the Queensway Carleton Hospital Radiologists within 48 hours and then delivered via Canada Post to your referring physician/s who sequentially, will provide you with the results.
Please book your follow up appointment with your family physician one week post examination.
Results may be faxed upon request if required.

SUMMARY:
Carleton Place and District Memorial Hospital Diagnostic Imaging staffs are committed to providing the best quality care possible to all of our patients.
Each of our technologists and sonographers are registered with our associated colleges such as:
CAMRT- Canadian Association of Medical Radiation Technologists
CMRTO- Canadian Medical Radiation Technologists of Ontario
CARDUP- Canadian Association of Registered Diagnostic Ultrasound Professionals
Our staff participate in continuing education programs to ensure that a high standard of radiographic service is maintained. Continuing education is also a requirement for our technologists to retain their individual licenses.
Our waiting room contains a folder filled with pamphlets. Please help yourself. A red binder titled “Patient Education’ is also available in the waiting room. Our clerical staff will be pleased to provide a copy upon your request.
Universal standard precautions are used in the Diagnostic Imaging Department.
Our department follows a stringent hand washing protocol. Signage is posted for staff and patients on proper hand-washing technique.
Patients will be asked to don a mask when presenting with a cough and fever.
All needles and syringes are single use only and are discarded immediately after use in a safe manner. Gowns, drapes, and surgical masks are single use. Drapes, sheets and pillow cases are outsourced for appropriate cleaning.
Our department follows the Spalding Chart of Disinfection.

Carleton Place and District Memorial Hospital is committed to providing the best in radiological services to our patients.
This commitment includes ensuring our equipment is maintained, serviced and tested on a regular basis.
Our department complies with the Healing Arts and Radiation Protection Act and our staff follow the ALARA principles. (as low as reasonably achievable)
All staff are trained to use our modern equipment and continue to educate and train on new techniques to ensure a high standard of care is maintained.
Suspected or known pregnancies:
Technologists will not radiate a fetus unless a physician has deemed the examination necessary, the patient has been educated on the risks, and provides the technologist with verbal and written informed consent.
We highly recommend all female patients to book their appointments within the first 10 days of her menstrual cycle. (please note: day 1 begins the day menses commences)
Contrast Injections for IVPs
Our department uses Omnipaque for contrast media when performing IVPs.
Before the examination, you must notify our staff of any history of allergic reactions including reactions to iodine and/or shell fish, asthma, diabetes, kidney failure or heart disease. Our brochure will give you detailed information on the examination.

Protecting the Privacy of Your Personal Health Information
We understand that health information about you is personal and must be protected.
For these reasons we will:
- Provide the highest level of confidentiality around the collection, use and disclosure of your personal health information;
- Collect only necessary information and use that information solely for the care and treatment you are seeking;
- Disclose only information necessary for the delivery of your care and the management of the health care system. This could include sharing information with your Family Doctor, a laboratory, another health care provider or your health insurance plan. This could also mean disclosing information to comply with legal and regulatory requirements, research requests, patient satisfaction surveys, fund raising and teaching or statistical requirements;
- Ask your permission before disclosing any of your information for purposes not related directly to the above;
- Recognize your right of access to your health information when you request it and provide copies for a fee;
- Be available to respond to your questions or concerns about the way we handle the privacy of your personal health information.
